A Collaborative Model for Enhanced Elder Care
This presentation aims to take you on a journey of cross-sectoral partners who all want to improve the care of elderly people and ensure that the best care possible is provided in the most appropriate setting for their needs. A mixed method evaluation of consumer satisfaction with New Zealand general practice nurses
Internationally, the rise in chronic and complex disease is prompting governments to build primary care nursing services to meet the growing burden of disease. Whilst practice nursing has been established in New Zealand for many years, the Ministry of Health has recently implemented a number of policies in order to increase multidisciplinary service provision and realise the potential of primary care to deliver personalised services to meet individual patient needs. It is timely, therefore, to investigate consumer satisfaction with practice nursing services in order to establish the acceptability of the service and highlight areas for improvement. A specialist wound clinic in general practice does it work
A presentation by Jan Rice at the 2013 APNA National Conference - Movers and Shakers.
After working in the Austin Health Would Clinic for over 14 years Jan Rice became aware of the need to try to bring the specialist to the patient and not the patient to the specialist.

Adult Vaccinations
Continuing improvements in health care, nutrition and disease prevention have led to ageing of the population, a trend which is set to continue. Older age of retirement and greater reliance on older adults for economic sustainability will drive incentives for health ageing. Vaccines are low hanging fruit for healthy ageing, yet under-utilized. There is a gap between rates of funded infant and elderly vaccination which is leaving older adults susceptible to preventable infections. Adult vaccination is a growing area of need. This includes the elderly and at-risk non-elderly adults in various risk categories such as travel, and with specific medical risk factors for disease. The elderly have a higher incidence and greater severity of vaccine-preventable diseases, due to both immunosenescence and co-morbidity, and represent the largest group with age-based recommendations for vaccines such as influenza, pneumococcal and herpes zoster. Whilst the number of available vaccines for the elderly and the need for these vaccines has increased, health provider attitudes to elderly vaccination remain negative. Reasons for this, including ageism in healthcare, will be reviewed, and solutions discussed. The need for a new approach to elderly vaccination will be reviewed.

An Update on the Regulation of Nurses and Midwives in Australia

Creating a high performing health system Integrated Care in Canterbury New Zealand
Does this sound familiar: A NZD$17m (AUD$16m) deficit, rapidly rising demand, a hospital frequently in gridlock, an ageing workforce, and shortages of staff? HPV and the Renewal of the National Cervical Screening Program
The national cervical screening program, which has been in place in Australia since 1991, is undergoing major changes set to be implemented from 1st May 2017. The starting age, interval and primary test for cervical screening are all changing. Knowledge, Attitudes and Practices Regarding Influenza Vaccination
An online survey of General Practice Nurses was conducted to ascertain the knowledge, attitudes and practices regarding influenza and influenza vaccination. The quantitative data was analysed using a combination of descriptive and inferential statistics.
The study found a high level of knowledge amongst GPNs related to seasonal influenza and demonstrated that this knowledge was related to vaccination status and risk perception.Knowledge levels and rates of vaccination uptake of H1N1 demonstrated an area in need of improvement. The findings are an important contribution towards guiding future efforts to both increase influenza vaccination uptake amongst nurses and enhancing the support provided to consumers.

During our participation in the 2013 Nursing in General Practice (NiGP) Demonstration Project, Illawarra Shoalhaven Medicare Local identified the need to improve engagement with nurses to enhance primary care nurse leadership and mentoring. To meet this need, leadership education was provided in collaboration with professional and academic partners. Additionally, a mentoring circle model reduced nurse isolation and facilitated knowledge and skills sharing. The mentoring circle brought together a group of highly motivated nurses from across the region. The networking meetings have helped reduce feelings of isolation and have empowered nurses to enhance their roles and identify the potential for change and improvement in their clinical setting.

My Kidneys My Health Empowering People with Early Stage Kidney Disease
1.7 million Australian adults have biomedical signs of early chronic kidney disease (CKD). Early detection and optimal management can reduce the decline in kidney function significantly. Limited educational materials exist that are health literate and empower self-management reducing risk of disease progression.
This session will share the results of a 14-item questionnaire to identify the information needs of people newly diagnosed with CKD to develop a consumer-driven resource pack containing information, advice, support and recommendations for early CKD.

My Story Caring for Syrian Refugees in Iraq
This keynote will outline Kristen's experiences working in northern Iraq, Kurdistan; where she joined with MSF to provide primary health care activities to Syrian refugees fleeing the Syrian conflict. She will speak about the refugee camp she visited where MSF had set up a medical project in order to support the health care needs of the growing community. National Health & Medical Research Council Overweight & Obesity Guidelines for General Practice
In 2010, the Australian Government's Department of Health and Ageing commissioned the NHMRC to develop Clinical Practice Guidelines for the Management of Overweight and Obesity for Adults, Adolescents and Children in Australia.
Intended for use by clinicians including general practitioners, primary health care nurses, primary health care professionals and allied health professionals, the Guidelines follow the primary care '5As' framework: ask and assess, advise, assist, arrange. A range of health benefits are promoted in the guidelines including healthy eating plans, increased physical activity and behavioural modification to help patients manage obesity.
The Guidelines make recommendations regarding the management of individuals who have a body mass index (BMI) greater than 25.0 kg/m2 and are at risk or currently have an obesity related comorbidity.

Dr Richard Hatchett, a nurse tutor from London, shares the UK's experience with Australian colleagues by exploring the wider factors that appear to push this form of healthcare delivery and assesses clinics' effectiveness and role in modern healthcare. Promoting Health in Prison
Prisoners have higher reported levels of risky behaviours and poorer health. Whilst holding a Men's Health Day is not a new concept in the community, in a prison it was, so there were many unique challenges to overcome.
Internal and external partnerships were formed to provide health information, health screening and healthy choices on the day.
Providing general information to larger groups has meant we have been able to identify health issues with some prisoners who would not normally access the health service on a regular basis.
Positive feedback from prisoners, Corrections Victoria and community agencies has encouraged the health team at Dhurringile to continue to run this activity annually.
Other facilities are looking at developing similar activities as part of Correct Care Australasia's commitment to prisoner health promotion and disease prevention.

Sexual health competency standards for primary health care nurses
General practice is the main provider of sexual health care in NSW. Fifty-five percent of Australians diagnosed with a sexually transmissible infection (STI) or blood borne virus (BBV) access treatment through general practice while eight percent access treatment through publicly funded sexual health services. The presence of symptoms is one of the main reasons people seek sexual health services. However, the asymptomatic nature of most STIs means that most people are not likely to be tested nor seek testing.
To address this, opportunistic STI screening should be a routine part of any medical consultation in general practice. The NSW Sexually Transmissible Infections Programs Unit (NSW STIPU), in partnership with the Australian Practice Nurses Association (APNA), the Australasian Society for HIV Medicine (ASHM), Family Planning NSW and the Australasian Sexual Health and HIV Nurses Association (ASHHNA) surveyed primary healthcare nurses on experiences with sexual health care in general practice. These survey responses indicated a high level of interest in sexual health and highlighted the value of developing sexual health competency standards for primary care nurses.

Striving to Ensure Quality Use of Medicine in Primary Health Care in Remote Australia
Nurses form the backbone of primary care in rural and remote Australia, providing an expanding volume of services, especially in relation to chronic disease management. They practise at an advanced level often with limited or distant medical support. The extended scope of practice requires nurses in the remote context to have a broad knowledge base in disease management, including the administration, monitoring, supply and storage of medications. The extended scope of practice of nurses in the remote context, and the additional challenges this presents in ensuring QUM, highlights the need for advanced knowledge and skills management. The Centre for Remote Health provides a course in the practical use of medicines in disease management aimed at ensuring medicines are used appropriately, effectively, judiciously and safely, called Pharmacotherapeutics for Remote Area Nurses. The Essential Role of Primary Health Care Nurses in Caring for People Approaching End of Life
Nurses represent the heart of primary health care. They provide a wide range of professional skills integral to holistic care. Increasing numbers of people with progressive conditions, both malignant and non-malignant, that are likely to lead to death in the near future are being cared for by primary health care teams. Nurses are optimally positioned and skilled to contribute to and coordinate the care of this vulnerable population. This talk will focus on a framework of care that nurses can help embed into everyday primary care practice that allows for routine recognition of people approaching end of life so that patient-centred and proactive management plans can be developed and implemented. These plans avoid decision making in emotionally charged situations and contribute to a quality death. The Impact of a Nurse led CDM - the Patients' Perspective
This presentation will present a mixed methods study which was conducted to examine the impact of nurse-led care in three general practices, in two states of Australia. Patients were randomised to either usual (general practitioner) or practice nurse led care. This presentation will report on the findings from the qualitative phase of the study.
Semi-structured interviews were conducted with ten participants purposively selected from the practice nurse led care group from each of the three general practices. Thematic analysis of interview transcripts was conducted to identify common threads that captured the essence of the participants' perceptions of the practice nurse-led care and the impact it had on their health and quality of life.

The Ninja Nurses of Primary Care
Some legendary abilities of Ninja warriors include invisibility, walking on water and control over the natural elements. Heather Henry, a former practice nurse and now the new chair of a forward thinking British primary care membership body, the NHS Alliance, thinks that primary care nurses are the uncelebrated 'ninjas' of their breed. Their invisibility is the thing she is tackling and she says myths have grown up about what they actually do, which stop young nurses coming into the specialism: 'Handmaidens', 'Marking time to retirement', 'Need years of experience', 'Part-timers doing the job for pin money', etc.
Heather and fellow nurse Louise Brady set up a national GPN network. They decided to use social media - Twitter and Facebook - to explain all that is fabulous about primary care nursing. A recent social media campaign had a social reach of over a quarter-million people.
Heather believes that primary care nurses are the beating heart of their communities and, as a social innovator, she has ideas about how they can develop their role. Heather's approach is often 'why have a meeting when you can have a party?' and she sees residents as equal partners and often teachers too.
Across the Commonwealth there is an opportunity to celebrate and share the Ninja skill. Heather thinks that not only must nurses unite, but our key assets, our GPs and patients must stand with us. For they know us best and their war cry is loud.

Using a Doppler to Calculate ABPI

What makes the Australian Medicare Local Alliance primary care nursing initiatives work...
The Australian Medicare Local Alliance (AML Alliance) has funded 12 Demonstration Projects within Medicare Locals and a series of Leadership in Action workshops, both designed to build the capacity and sustainability of primary health care nursing.
The Australian Health Workforce Institute (AHWI) at University of Melbourne has been funded to identify what makes the AML Alliance nursing initiatives work, for whom and in what circumstances, and to explore how to build the capacity of Medicare Locals to support primary care nursing. A formative evaluative approach informed by a realist evaluation framework is being used.
The evaluation involves the collection of qualitative data via semi-structured interviews with participants and key stakeholders involved in the funded initiatives. Preliminary evaluation findings will be presented with the aim of informing policy and practice decision-making to build the capacity and sustainability of primary health care nursing at the Medicare Local and broader health system level.
